Please complete and return the Report of Medical History [PDF 266KB] by June 30 if you are enrolling in the fall and by December 30 if you are enrolling in the spring.

New York State law requires all students to provide the Student Health Center with physician-certified evidence of immunity to measles, mumps, and rubella. All vaccinations must have been given after one year of age and after January 1, 1968.

Rensselaer is required by the provisions of this law to refuse to allow you to continue in attendance if you have not supplied this evidence. In addition, Rensselaer requires proof of a tuberculosis test within the past year in the United States, a tetanus vaccine within the past 10 years, and, for those students under 35, a meningitis shot within the past 3 to 5 years.
